    • 1). Contact the local Human Services office to get an application. In some states, the Human Services office may be called Social Services or Family Services. It is usually easier to go to the office and pick up an application rather than calling to get further information. While picking up the necessary paperwork that will need to be filled out, it may be possible to ask a few general questions. Some states have websites that give economic guidelines and where an applicant can apply online. It would be wise to apply anyway, even if the income seems to be over the stated guidelines. Depending on the states laws, other factors such as how much is paid for heating, is considered while qualifying applicants.

    • 2). Gather the documentation that will be required for filling out the application. Whether the public assistance application is completed online or in paper form, there will be some information necessary to complete the forms. Documentation such as Social Security cards of every member of the household, driver's licenses, the previous year's income tax return and income for the last month for every person receiving money. Make copies of each of these documents plus any others that are listed in the application. These copies will be turned in at the time of the appointment review.

    • 3). Fill out the public assistance application using the information that was copied. Make sure that the application is filled out correctly since this is a government document that will be signed at the end stating that the information is correct.
