How Do I Obtain a Tennessee Driving Record?
- 1). Obtain a "Vehicle Information Request" form from the Tennessee Department of Revenue Taxpayer and Vehicle Services Division. This form can be obtained at the department's website or by making a request by mail.
- 2). Complete Section A. This section requests the name and contact information about the person requesting the driver's history.
- 3). Complete Section B. This section requires a choice from four options that address the type and cost of obtaining the desired records. Additionally, you can provide a description of the driver's vehicle, although this is not required for obtaining the driver record.
- 4). Complete Section C, if it applies to you. This section is meant for government entities that are requesting the information. Generally, government agencies do not need to pay a fee to obtain these records.
- 5). Complete Section D. This section requires the requesting party to indicate a legal reason for requesting the driver history. This is required in order to comply with the Federal Driver's Privacy Act. Lawful reasons include obtaining the records for litigation because of an automobile accident; requests by accident claims representatives or by private investigators who have a bona fide purpose for requesting the information; and cases in which the driver has consented in writing to release the information to others. If the driver has given consent, he or she must complete a signed, notarized request.
- 6). Sign and date the request form. You must also provide your driver's license number and state of issuance.
- 7). Photocopy the completed forms and keep a copy for your files.
- 8). Send the completed form, along with a copy of your driver's license and a check or money order for the appropriate fee, to the Tennessee Taxpayer & Vehicle Services Division.
